How to Reverse Fatty Liver: What the Evidence Shows

The short answer

For many people, fatty liver can be improved and even reversed, but only by fixing the metabolism behind it, not with a detox tonic or a cleanse. The strongest evidence is for weight loss: in a 2015 Gastroenterology study, patients who lost at least 10 percent of their body weight had steatohepatitis resolve in 90 percent of cases and early scarring regress in 45 percent. A Mediterranean, plant-predominant diet and cutting sugary drinks help too, and for moderate to advanced disease there are now two FDA-approved medicines, resmetirom and semaglutide. Simple fatty liver often improves, but reversal of scarring is not guaranteed, and your fibrosis stage decides whether you are in lifestyle territory or medication territory.

What we cover

·      What fatty liver, now called MASLD, is, and why it affects about one in three adults (Younossi, Clinical and Molecular Hepatology, 2024)

·      Why losing at least 10 percent of body weight resolved steatohepatitis in 90 percent and regressed early fibrosis in 45 percent in a 2015 Gastroenterology study (Vilar-Gomez, 2015)

·      Why a Mediterranean, plant-predominant plate and cutting sugary drinks lower liver fat, because fructose pushes the liver to make new fat

·      The two new FDA-approved medicines for MASH with moderate to advanced fibrosis, resmetirom (Rezdiffra) (MAESTRO-NASH, 2024) and semaglutide (Wegovy)

·      The ESSENCE trial result: steatohepatitis resolution in 62.9 percent on semaglutide versus 34.3 percent on placebo (Sanyal and Newsome, 2025)

·      Why milk thistle, bitter tonics, and detox teas lack high-quality trial evidence for reversing fatty liver or fibrosis

·      Why a real diagnosis with an ultrasound or FibroScan comes first, because your fibrosis stage sets the plan

When to see a doctor

If you have fatty liver plus diabetes, heavy alcohol use, or signs like yellowing skin or swelling in your legs, get evaluated promptly rather than self-treating.
