A parent or legal guardian completes this stage-one application for a Southern California student in grades 4 through 8. It collects only the information needed to review initial eligibility and contact the family.

In the student's own words, write about 500 words. Describe the sport you play, why it matters to you, how it has shaped your life (such as confidence, friendships, health, teamwork, or goals), and what continuing to play would mean to you. A parent or guardian may type the student's response here. Please avoid medical, financial, or other sensitive details.
